Dental implants are screws, usually made of titanium, that are inserted into the jawbone to treat cases where one or more teeth have been lost due to various reasons.Implants can be used not only to replace missing teeth by applying fixed prostheses, but also to ensure the stability of removable prostheses.
Any patient without general health problems or medication use that would prevent implant placement is a suitable candidate. In individuals with chronic conditions (diabetes, hypertension, thyroid problems, etc.) and those who have previously undergone surgery, implant placement may be appropriate after consultation, taking into account the patient's condition and medications.
This treatment is suitable for individuals who do not have general health problems that could affect the success rate of the treatment, and who maintain good oral health through proper habits and attention to oral hygiene (e.g., gum disease). In addition, patients should not have irreversible bone loss in their jawbones.
In cases where general health problems may prevent surgical procedures, an opinion is first sought from a specialist physician, and the patient's overall health is primarily considered when planning treatment. If the patient's general health is not suitable for implant surgery, treatment cannot be performed, or treatment is delayed until the patient's general health is brought under control. In addition, if any medication is being used that could affect treatment success, these medications should be discontinued or interrupted (see blood thinners) under the supervision of a doctor before treatment. If this is not possible, implant procedures cannot be performed. For successful implant placement, the jawbone must have a certain quantity and density. If the necessary amount of bone is not available and severe gum disease is present, implant treatments can only be performed after these diseases are eliminated and sufficient bone formation is achieved (see bone graft).
Dental implant treatments consist of two stages: surgical and prosthetic. The first stage is the surgical stage, where implants are placed in the jawbone. This stage is performed using either an open or closed method, depending on the individual's jaw structure and the determined treatment plan. In open implant surgery, stitches are required in the gum after the procedure, whereas in the closed method, no stitches are needed in the area where the implant is placed. After the surgical stage, depending on the condition of the jawbone, it may sometimes be possible to immediately proceed with the prosthetic stage, meaning placing prosthetic teeth on the implants. However, in most cases, it is necessary to wait for the jawbone to heal. Jawbone healing usually takes between 3 and 6 months. If no problems are observed after this period, the prosthetic stage is completed, and the prostheses are permanently placed on the implants.
In suitable cases, the tomography results provide a 3D image of the jawbone, which is then used to create a treatment plan using computer software, and appropriate surgical guide plates are produced. These guide plates show the exact location of the implant in the jawbone. Thus, flapless, or sutureless, surgical procedures, also known as "closed technique," are performed. In the area indicated by the guide plate, only the portion of the gum where the implant can be placed is opened like a flap, eliminating the need for sutures after the procedure.
During the bone healing process following implant placement, it is important to avoid putting stress on the jawbone and to prevent the patient from losing their teeth. Therefore, sometimes temporary prostheses or dentures are made for the patient, and sometimes existing dentures are used as temporary prostheses by adapting them to avoid putting stress on the jawbone.
The duration of surgical procedures can vary depending on whether another surgical procedure is performed along with the implant, or the number of implants required. In some cases, a dental implant surgery can be completed in as little as 15 minutes. However, in more complex cases and when multiple implants are being placed, the surgical procedure can take several hours.
In cases where single tooth deficiencies are addressed with bridge treatments, implant applications, which eliminate the need to grind down healthy teeth adjacent to the missing tooth, can be cited as an example of the beneficial application of implant procedures.Furthermore, removable dentures, used in cases of multiple missing teeth or complete tooth loss, may not provide sufficient satisfaction in terms of aesthetics and comfort. With implant applications, individuals can have fixed teeth again, which is one of the most important advantages of implants.Implants can sometimes be used in conjunction with removable dentures. In this case, another benefit of implant placement is that the removable dentures remain stable during chewing and speaking.
The first and most important condition for the long-term durability of implants is maintaining good oral hygiene. Without proper oral care, the lifespan of implants in the mouth will be reduced, just like that of natural teeth.Furthermore, the correct placement of the implant using the right materials and by a skilled physician with the correct techniques is of great importance for both the success of the treatment and the lifespan of the implant.
Maintaining the highest level of oral hygiene is crucial during the recovery period after implant surgery. Furthermore, it is very important to strictly follow your doctor's recommendations and avoid putting excessive stress on the jawbone.Avoiding tobacco products like cigarettes during such times is very important for the recovery period.
Implant treatments are not inexpensive because both the implant material is made from high-quality, mass-produced materials and the application process requires specialized expertise. However, when the quality of treatment and post-treatment patient care are taken into consideration, they are not considered expensive treatments.
General X-ray techniques only provide a 2D image of the jawbone. However, using tomography, a 3D image of the jawbone, the most realistic representation of its shape, is obtained. Therefore, more accurate treatment planning can be done with tomography. In some cases, implant procedures can be performed without tomography, but it is of paramount importance for the most accurate treatment methods and planning.
Smoking is a habit that negatively affects oral health in general. Smoking after implant surgery negatively impacts bone healing and significantly reduces the chances of successful treatment. Therefore, if a person smokes or uses other tobacco products, it is strongly recommended that they quit this habit before treatment.
